My will at the end of life
The purpose of this brochure is to provide citizens with impartial information about the laws governing the end of life and the options available to them. With the aim of ensuring that the wishes of a person at the end of life are respected, even if they can no longer express themselves, part of the brochure consists of ‘advance directive’ and ‘end-of-life provision’ forms, which can be used by citizens to document their choices. Any patient can complete both an advance directive and an end-of-life provision.

Ma volonté en fin de vie
Cette brochure vise à informer de façon impartiale les citoyens sur les lois encadrant la fin de vie et les options qui sont à leur disposition. Dans l’objectif de faire respecter la volonté d’une personne en fin de vie, même si elle ne peut plus s’exprimer, une partie de la brochure est constituée de formulaires de « directive anticipée » et de « disposition de fin de vie », qui pourront être utilisés par le citoyen afin de documenter ses choix. Tout patient peut remplir à la fois une directive anticipée et des dispositions de fin de vie.
Mein Wille am Ende des Lebens
Diese Broschüre soll die Bürger unparteiisch über die Gesetze, die das Lebensende regeln, und über die ihnen zur Verfügung stehenden Optionen informieren. Mit dem Ziel, den Willen einer Person am Lebensende zu respektieren, auch wenn sie sich nicht mehr äußern kann, besteht ein Teil der Broschüre aus Formularen für eine „Patientenverfügung“ und eine „Verfügung am Lebensende“, die von den Bürgern verwendet werden können, um ihre Entscheidungen zu dokumentieren. Jeder Patient kann sowohl eine Patientenverfügung als auch eine Patientenverfügung ausfüllen.
